This can include your medical history, medications, adverse events, hospital/clinical history, notes of your symptoms or diagnosis and treatments given to you, any specialist reports/test results and your work history/lifestyle activities.

Further, when we collect hospital records, sometimes this information may contain your Medicare number and/or private health insurance details.

directly from you where it is reasonable and practicable to do so. This might be via a face-to-face discussion, telephone conversation, registration form or online form via the Website; and/or
from the person or organisation who referred you to our services, such as your solicitor or employer (as part of the referral process).
