At Axiom Health Pty Ltd (Axiom, we, us, our) we value your privacy and take all reasonable steps to ensure your information is protected from misuse and unauthorised disclosure.

This Privacy Policy describes our practices in collecting, using, disclosing, retaining, and protecting the information that we or our service providers collect, including when you visit www.axiomhealth.com.au (the Website).

We have adopted the Australian Privacy Principles (the APPs) contained in the Privacy Act 1988 (Cth) (the Act). The APPs govern the way in which we collect, use, disclose, store, secure and dispose of your personal information.

We further have developed this Privacy Policy to comply with specific privacy obligations in accordance with the Health Records and Information Privacy Act 2002 (NSW) (the HRIP Act).

This Privacy Policy uses the definition of “Personal Information” and “Health Information” which is set out in the Act and the HRIP Act.

A copy of the APPs may be obtained from the website of The Office of the Australian Information Commissioner at www.oaic.gov.au. A Copy of the HRIP Act can be obtained from the Information and Privacy Commission of New South Wales at www.ipc.nsw.gov.au.

Axiom adheres to the APPs, the Act and the HRIP Act.

1. Collection of Personal Information

Axiom will, from time to time, receive and store Personal Information that you enter on the Website, provide to us directly, or as given to us in other forms.

The type of Personal Information we may collect, and hold, includes your name, address, date of birth, email and other contact details. Additionally, we may also collect any other Personal Information you provide whilst interacting with us.

When Axiom is engaged to provide our professional services we may collect Personal Information, including Health Information, which is provided by the person or organisation who referred you to our services, such as your solicitor or employer. This can include your medical history, medications, adverse events, hospital/clinical history, notes of your symptoms or diagnosis and treatments given to you, any specialist reports/test results and your work history/lifestyle activities.

Further, when we collect hospital records, sometimes this information may contain your Medicare number and/or private health insurance details.

2.  How we collect your Personal Information

Axiom generally collects Personal Information in the following ways:

  • directly from you where it is reasonable and practicable to do so. This might be via a face-to-face discussion, telephone conversation, registration form or online form via the Website; and/or

  • from the person or organisation who referred you to our services, such as your solicitor or employer (as part of the referral process).

3.  Use of your Personal Information

Axiom uses your Personal Information solely to provide our services to you and to the person/organisation that referred you to us (such as your solicitor or employer).

We may also use your Personal Information to comply with our legal obligations.

Other than in the course of providing our services, or as otherwise described in this Privacy Policy, Axiom will not share your Personal Information with any third party without your consent.

4.  Disclosure of your Personal Information

Axiom may disclose your Personal Information:

  • to any of our employees, officers, insurers, professional advisers, agents, suppliers, or subcontractors insofar as reasonably necessary for the purposes set out in this Privacy Policy. Personal Information is only supplied to a third party when it is required for the delivery of our services;

  • to the person or organisation who referred you to our services (such as your solicitor or employer);

  • we may, from time to time, need to disclose Personal Information to comply with a legal requirement, such as a law, regulation, Court order, subpoena or warrant in the course of a legal proceeding or in response to a law enforcement agency request; and/or

  • with the consent of the individual concerned.

Axiom does not sell or rent any of your Personal Information to others. Axiom does not disclose any Personal Information outside of Australia.

5.  Acceptance

By providing us with your Personal Information you hereby accept and consent to the terms of this Privacy Policy and the types of disclosure covered by this Privacy Policy.

6.  Security of your Personal Information

Axiom is committed to ensuring that the Personal Information you provide to us is secure. We will hold your Personal Information for as long as you authorise us to. We adopt appropriate data collection, storage and processing practices and security measures to protect against unauthorised access, alteration, disclosure or destruction of your Personal Information stored by us.

We will take reasonable steps to destroy, permanently delete or de-identify any Personal Information that is no longer required for any purposes described in this Privacy Policy.

If there is a security issue/breach, Axiom will investigate suspected breaches to ensure that the cause is identified and corrective actions are implemented. Where applicable, we will notify affected individuals and the Office of the Australian Information Commissioner.

7.  Non-Personal Identification Information

We may collect non-personal identification information about you whenever you interact with the Website. Non-personal identification information may include the browser name, the type of computer and technical information about your means of connection to the Website (such as the operating system and the internet service providers you use) and other similar information.

As with most websites, we may use "cookies" to enhance your experience. Cookies do not cause damage or harm your system and are used to assist you with the Website. The Website may, from time to time, have links to other websites not owned or controlled by us. These links are meant for your convenience only. Links to third party websites do not constitute sponsorship or endorsement or approval of these websites. Please be aware that Axiom is not responsible for the privacy practices of other such websites. We encourage our users to be aware that when they leave the Website, they should read the privacy statements of each and every website that collects Personal Information.

10. Updating or Correcting your Personal Information

Your Personal Information needs to be up to date for us to perform our services. For this reason, it is important that any information we collect is accurate, complete and up to date.

If any of your Personal Information changes, we ask that you notify us in writing at admin@axiomhealth.com.au.

11.  Access and Complaints

You may access your Personal Information by making a request in writing at admin@axiomhealth.com.au, detailing the information you are requesting and providing sufficient proof of identity.

If you have a concern or complaint, please contact us in writing at admin@axiomhealth.com.au. We will respond to your concern within thirty (30) days.  

12.  Changes to this Privacy Policy

Axiom has the discretion to update this Privacy Policy at any time. All modifications will be effective immediately upon our posting of the modifications on the Website. You should visit the Website regularly to review this Privacy Policy.
