Vocational Assessment (VA) Report
The Axiom Vocational Assessment (VA) report assesses the functional impact of injury on work capacity; key evidence for economic loss, TPD, superannuation and income protection claims. Physical and psychological injuries are assessed by considering functional capacity in the context of the core tasks of any given occupation.
Opinion is provided to determine a client’s residual work capacity (if any) following injury, having regard to their transferable skills, qualifications and work experience. Occupation-specific earnings information is included to assist in quantifying damages.
Activites of Daily Living (ADL) Report
The Axiom Activities of Daily Living (ADL) report assesses the functional impact of personal injury on everyday activities, such as personal care, domestic and yard maintenance activities, leisure activities, community access and childcare tasks. We assess and quantify past gratuitous care and future care needs. This report also assesses and quantifies assistive equipment, home modification and specialist therapy needs.
The Axiom Activities of Daily Living (ADL) report includes detailed calculation schedules, ensuring claims are valued in accordance with legislative requirements.
Compensation to Relatives Report
The Axiom Compensation to Relatives report quantifies loss experienced by a surviving family member consequent to the untimely passing of the deceased. We assess, quantify and cost the contribution that would have been provided by the deceased to the household (for tasks such as childcare activities, domestic tasks and yard maintenance) but for their untimely passing.
This report also quantifies past and future requirements for care in the event the surviving family member has suffered a psychological injury consequent to the passing of the deceased.
NDIS Functional Capacity Assessment Report
The Axiom NDIS Functional Capacity Assessment Report provides a detailed functional capacity evaluation of applicants seeking access to, or review of existing services from, the National Disability Insurance Scheme (NDIS). An applicant's capacity to undertake everyday activities in the domains of COMMUNICATION, SOCIAL INTERACTION, LEARNING, MOBILITY, SELF-CARE and SELF-MANAGEMENT is assessed and recommendations are made to maximise safety and independence. Our detailed reports include clarification as to whether impairment(s) result in substantially reduced functional capacity to undertake daily activities. This report provides valuable clinical assessment for matters before the Administrative Appeals Tribunal.
Video Evidence: Day in the life of your client
Axiom appreciates that the context of the home environment is a key factor in understanding the impact of injury on an individual. The environments in which people live vary enormously, as does the support structures in place. Accordingly, we have identified a need for the provision of objective video evidence to complement our written reports.
Our “Day in the Life” video evidence is undertaken with full regard for specific client requirements in relation to religious or cultural considerations and to privacy.
Life Care Plans (catastrophic injury)
The Axiom Life Care Plan is suitable for catastrophic injury claims, such as traumatic brain injury or spinal cord injury, where a regular Activities of Daily Living Report may be insufficient to adequately quantify client care needs. Please discuss any catastrophic claims with us so we can determine how best to assist you. This report is charged at an hourly rate according to complexity. We will quote for this report at the time of referral.

Family Law Occupational Therapy Report
The Axiom Family Law Occupational Therapy Report provides assessment of present and future requirements for care, domestic assistance, vocational capacity, assistive equipment and therapy services pursuant to Section 75(2) of the Family Law Act 1975.
Detailed Calculation Schedules
Axiom Activities of Daily Living (ADL) reports, Compensation to Relatives reports and Life Care Plans include detailed calculation schedules, ensuring the recommendations are quantified according to legislative requirements.

Supplementary report
Axiom occupational therapists will promptly prepare supplementary reports where client circumstances have changed, additional medical information becomes available or to respond to opposing occupational therapy opinion. The original assessing occupational therapist will be commissioned for this work.
